Subjective information(S):

(Did you say what you wanted to talk about to Doctor D ?) = (Yes,) I did.

(Often nods to our questions. Speaking in a small voice) I'm hungry.   (she said 2 times)

 

Objective information(O):

Very dry lips.

Strong feeling of weakness, a vacuous expression. But she isn't sleeping, she always says something, moving her neck. She can nod to our questions, but cannot talk loudly voice.

 

She could say only short words.

They were aware that her lips were very dry and she was very weak.

They didn’t check or measure her blood glucose level. Despite the need to take immediate action when her body showed abnormalities and side effects.

As usual, they only measured her blood pressure and pulse and temperature. The normal inspection(check or measure) was 4 times a day, but for several days it was twice a day.

 

The blood pressure and pulse were not measured.

And their actions were only that they wrote 「Weak and unable to measure」.

 

We don't know what Doctor D or nurse told the staff or nurse about the drug's side effects and about discontinuation of administration.

 

And we don’t know and don't understand.

Why didn’t Doctor A or Doctor D or the nurse didn't recognize and pay attention to the side effects of the drugs?

 

Also we don’t understand.  Why did they give her a psychiatric exam but not the prompt physical care?
